Right-to-Know Request Form
The completed form shall be submitted to Right To Know Officer Cheryl Gibson, Room 217, Municipal Building; e-mail firstname.lastname@example.org; fax 717-228-4450.
The City of Lebanon will fulfill a request for records within five business days from the date the request is received, or provide written notice that the request is being reviewed or it has been denied.
Reasonable fees as recommended by the Pennsylvania Office of Open Records may be levied for postage, duplication, certification, conversion of public records to paper, and certification of documents.
If a written request is denied, the requester may appeal to the Office of Open Records within 15 business days of the mailing date of the city’s response denying the records. Appeals should be mailed to the Office of Open Records, 333 Market Street, 16th Floor, Harrisburg, PA 17126-0333.
